The Leon County Clerk and Comptroller is a federally designated passport acceptance facility serving the public in Leon County and surrounding areas. A passport is an internationally recognized travel document attesting to the identity and nationality of the bearer. The passport is a request on the part of the issuing government that officials of foreign governments permit the bearer to travel in their territories and to afford them all lawful aid and protection.

A passport card is the wallet-size travel document that can only be used to re-enter the United States at land border-crossings and sea ports-of-entry from Canada, Mexico, the Caribbean, and Bermuda. The card provides a less expensive, smaller, and convenient alternative to the passport book for those who travel frequently to these destinations by land or by sea. The passport card is adjudicated to the same standards as the passport book. You must provide documents that establish your U.S. citizenship and identity.

Can a passport be renewed by mail?

Yes. If your most recent passport was issued when you were at least 16 years old, issued within the past 15 years, and you use the same name as on your most recent passport OR you have had your name changed by marriage or court order and can submit proper documentation to reflect the name changes. Mail your renewal application to one of the addresses listed on the DS-82 form.

Can you get a passport if a birth record is not available?

Yes. This requires additional documentation. You should call the Clerk's Office (850-606-4060) for specific instructions regarding your particular situation.

Do I have to apply in person?

Passport services require the personal appearance (1) of all children applying for U.S. passports as well as both parents, or (2) your passport was lost or stolen, or (3) your passport was issued more than 15 years ago, or (4) you met the requirements stated above for the DS 82 or 5504 applications.

Expedited Service for Traveling Within Two Weeks

If you are traveling within 14 calendar days, you must make an appointment at the nearest Passport Agency by calling the National Passport Information Center at 1-877-487-2778.

How do I report a missing passport?

Complete form DS-64 and bring it to the Clerk's Office when you apply for a new passport.

How long is a passport valid?

The passport is valid for ten (10) years if you are 16 years of age or older. The passport is valid for five (5) years if you are 15 years of age or younger.

What are the requirements for a passport?

New Applicants
You must complete the Passport Application Form DS-11 and apply in person if any of the following are true: SAVE TIME and complete your application prior to getting to our office by simply clicking on the link above. Just remember, DO NOT sign it until you are in front of one of our clerks.
  • You have never had a U.S. passport.
  • Your most recent passport was valid for five years only.
  • Your most recent passport was issued more than 15 years ago.
  • Your most recent passport was lost or stolen.
Please bring with you the following required documentation:

Proof of Citizenship (Provide one of the following):
  • Previous U.S. Passport
  • Certified copy of your birth certificate (must have the names of both parents on it)
  • Certificate of Naturalization
Proof of Identity (provide one of the following):
  • Previous U.S. passport
  • Valid driver license (A temporary or learner's license does not meet this requirement.)
  • State identification card (Photographic likeness should be recent.)
  • Certificate of Naturalization
If you cannot prove your identity as outlined above, please contact our office at 850-606-4060 for details on other options. One recent passport photo, 2" x 2" is required. There are also private photographic studios in Tallahassee that can provide this service.
Passport Renewal
All Passport renewals require the completion of Form DS-82. You may renew your passport by mail if (1) you hold a passport that was valid for 10 years and expired less than 5 years ago, and (2) your valid passport has not been lost or destroyed. The mailing address is listed on page 2 of the application form.

We have forms available and can answer any questions to assist you. We can also take the required photos; however, you are responsible for mailing the actual renewal application.

Instructions
  • Complete, sign and mail Form DS-82. The mailing address is on page 2 of the application form.
  • Include your most recent passport with one new passport photo. If the name you are using differs from the name on your most recent passport, you must submit a (1) certified copy of the marriage certificate, or (2) certified copy of court order showing the change. Photographs can be taken at the Clerk's Office at 1276 Metropolitan Boulevard, #101, for a $15 fee. Click here for our current hours of operation.   • If your passport is mutilated or damaged, you must (1) use the DS-11 application form and apply in person at our office at 1276 Metropolitan Boulevard, #101, Tallahassee. DO NOT sign the form until you are in front of one of our clerks.
Passport Reapplication
If your most recent passport was issued less than one year ago, use the DS-5504 form for the following:
  • Name change
  • Data correction
  • Limited passport replacement
We have forms available and can answer any questions to assist you. We can also take the required photos; however, you are responsible for mailing the actual application.

Instructions
  • Complete, sign and date the Reapplication Form DS-5504.
  • Mail your application to one of the addresses listed on the back of the DS-5504 form.
  • There is no fee if your passport was issued less than 1 year ago, unless you request expedited processing.
If your most recent passport was issued more than 1 year ago AND you have the passport, use Form DS-82.

Instructions
  • Complete, sign and mail your application to the address listed on the back of the DS-82 form.
  • Include the appropriate payment/fee.
If your most recent passport was issued more than one year ago AND you do not have the passport, complete Form DS-11 and bring it to the Leon County Clerk & Comptroller's Northeast Branch office at 1276 Metropolitan Boulevard, #101, for processing. DO NOT sign the form until you are in front of one of our clerks.
Application for Minors (under age 16 - for new and renewal passports)
Instructions

You must complete the New Applicant DS-11 form and include the following. SAVE TIME and complete the application prior to getting to our office by simply clicking on this link. Just remember, DO NOT sign it until you are in front of one of our clerks.

Proof of Child's Citizenship (provide one of the following):
  • Certified copy of your birth certificate (must show the names of both parents)
  • Certificate of Naturalization
  • Expired passport
Proof of Both Parent's Identity (provide one of the following):
  • Previous U.S. passport
  • Government issued identification card (photographic likeness should be recent)
  • Certificate of Naturalization
Proof of Parental/Guardian Relationship
  • Birth certificate listing both parents' names, or
  • Proof of adoption.
Both parents must appear together to sign the DS-11 form or one parent must be present to sign the application, and the absent parent MUST submit a completed and notarized "Statement of Consent" Form DS-3053 AND a photocopy (front and back) of their ID. The notarized consent form cannot be more than three months old.

When should I apply for a passport?

You should apply for a passport several months before you plan to travel. If you will need a visa from a foreign embassy, you will need to allow even more time. On average it takes 8-11 weeks to receive your passport.

Who needs a passport?

Anyone, including infants, departing from the United States and entering the United States from outside the United States. Travel.State.Gov contains information regarding the passport and visa requirements of foreign countries, and their respective fees.

A passport is not required for travel by U.S. citizens to any territory or waters, including Puerto Rico, Guam, American Samoa and the U.S. Virgin Islands.
